Medical
Curbside Pick-Up is available for Medical Patients at our Addison location beginning at 9:30AM and ends at 8PM Monday through Saturday. On Sunday, curbside is available from 10:30AM to 5PM.
Please note that the state of Illinois has currently extended Curbside Pick-up through 1/31/23. We will keep you updated as to an extension as soon as we are notified.

Curbside Etiquette
1. Please have both Med card and valid I.D. ready for budtender to take. Upon arrival, please call the store (630-519-1300) or tap “I’m Here” on your order.
2. Please have cash in hand or if you paid via Aeropay, please make sure to let the budtender know.
3. Please allow at least 10 – 15 minutes for a budtender to be present, both before and during the curbside.
4. When the budtender has dropped off your product, they will confirm with you that your IDs and change are in the bag.
Curbside ends an hour before we close, and any remaining curbside orders will have to be picked up indoor as an online pickup order instead.
Any resident of the state of Illinois can apply who has one of more the program’s qualifying medical conditions and does not have any criminal felony drug convictions. Children under 18 are also eligible.
Effective January 1, 2015, the Compassionate Use of Medical Cannabis Pilot Program Act was amended to provide registration identification cards for qualifying patients under 18 years of age, enabling them access to medical cannabis-infused food products for treating or alleviating symptoms associated with one or more debilitating condition. Applicants under 18 years of age must obtain certification from a physician with whom they have a bona-fide relationship and from a reviewing physician. Additionally, parent/custodial parent or legal guardian of the qualifying patient under 18 years of age must serve as the caregiver for the patient. Beginning January 1st through January 31st, and from July 1st through July 31st, Illinois residents may petition the Illinois Department of Public Health to add debilitating medical conditions or diseases to the Medical Cannabis Patient Registry Program. Petitions postmarked January 1 through January 31, and from July 1 through July 31 will be accepted for consideration. Petitions must be mailed with all required documents to the following address (petitions may not be submitted via e-mail):

A designated caregiver is a person who is selected by a qualifying patient as the person authorized on the qualifying patient’s behalf to obtain medical cannabis from a certified medical cannabis dispensary and to dispense and assist in the administration of medical cannabis. Caregivers must enroll in the program. A designated caregiver is issued a medical cannabis registry identification card that allows him/her to possess up to 2.5 ounces of medical cannabis on behalf of their patient. It is not legal for caregivers to consume, by any means, medical cannabis that has been dispensed on behalf of a registered qualifying patient. All necessary application forms may be submitted online or through the mail and can be found on the Forms page of our website and on the medical cannabis program website.
A qualifying patient who is a veteran and receiving medical care and treatment at a VA hospital must:
- Be an Illinois resident and provide two valid items proving residency.
- Have a qualifying debilitating medical condition.
- Provide a copy of his or her U.S. Department of Veterans Affairs official hospital medical records requested on VA Form 10-5345. If you have received care for your debilitating medical condition for more than 5 years at a VA facility, you must mark “OTHER” on VA Form 10-5345 under “INFORMATION REQUESTED” then specify that you are requesting information about the treatment of your debilitating medical condition for the most recent 12-month period. Under “PURPOSE(S) OR NEED FOR WHICH THE INFORMATION IS TO BE USED BY INDIVIDUAL TO WHOM INFORMATION IS TO BE RELEASED” write “personal medical purposes.” Under “NAME AND ADDRESS OF ORGANIZATION, INDIVIDUAL OR TITLE OF INDIVIDUAL TO WHOM INFORMATION IS TO BE RELEASED” write your address. Once you receive your official medical records, you must submit the medical records with your application.
- Provide a copy of his or her DD214 or equivalent certified document indicating character and dates of service.
- Complete the fingerprint-based background check and not have been convicted of an excluded offense.
- Submit a non-refundable application fee with the signed Registry Identification Card Application to the Illinois Department of Public Health Division of Medical Cannabis.

Create an account through the State of Illinois Medical Cannabis Pilot Program, submit your application including your certification form, and pay the associated fees. During this process, you will also need to designate your dispensary of choice. For more information on requirements & restrictions, visit: dph.illinois.gov.
In addition to the completed application, applicants will need to submit the following along with their application:
A. Non-refundable application fee payable by check or money order made out to the Illinois Department of Public Health. The fees for obtaining your medical cannabis card are:
- $100 for 1 year
- $200 for 2 years
- $250 for 3 years
B. 2” x 2” photograph taken in the last 30 days (shot on a plain, white or off-white backdrop in full color, directly facing the camera with a neutral facial expression & both eyes open, (we recommend you use a passport photo vendor to ensure the photograph meets these requirements.)
C. A completed copy of the Fingerprint Consent Form (fingerprints must be taken within 30 days of submitting an application), with the following supporting documents attached:
- Proof of age and identity (a color photocopy of either an Illinois Driver’s License, Illinois State ID, or the photograph page of a US passport
- Proof of Illinois residency: Proof of residency requires 2 documents that prove you live in Illinois
- Bank statements
- Utility bills
- A state ID
- A driver’s license
- A voter ID card
- Fingerprint receipt
- Benefit Verification Letter from the Social Security Administration or DD-214 (*only if applicable – this is for applicants who are applying with reduced fees)
Caregivers
A patient’s caregiver can register to get and transport cannabis to the registered patient. A caregiver must complete the entire caregiver application. They must send it with the $25 caregiver fee and all supporting documents, including:
- Photo
- Proof of residency
- Proof of age and identity
- Caregiver’s signature
- The caregiver application should be submitted with the patient’s application

#3 Visit a Dispensary
On February 19, 2019, IDPH began issuing provisional (temporary) access registration cards to online applicants. After applying, you can access & print your provisional card within 24-48 hours. It’s recommended that you call your designated dispensary to verify your card is active before driving to the physical location.

Patient Forms
Submit your application for a Medical Cannabis Registry Card online to receive provisional access to a licensed dispensary within 24 hours! Qualifying patients can also submit paper applications, but will not receive a provisional (temporary) registry card.
Minors & Caregivers
The Parent and or Guardian of a qualifying minor will have to fill out the Minor Application Form and Caregiver Application Form. Minors will also need to acquire a Viewing Physician Written Consent Form, as well as the standard Health Care Professional Written Certification Form.
Patients with Terminal Illness
Anyone who has received a diagnosis of terminal illness with a prognosis of less than six months of life will have their application fee waived. The Terminal Illness Application Form must be completed and can be submitted online to the IDH online portal.
Veterans
If you are a veteran receiving care from a Veterans Affairs facility, you will not need a physician to sign any forms on your behalf. Go to the Release of Information office at your VA to request your medical records for the previous year, as well as your complete DD-214 to certify your dates of service and condition of discharge. Include all of these documents with your Qualifying Patient application and submit by mail.

This notice explains how Hatch may use and share your information to provide services, obtain payment for these services, and operate our business. - To provide information related to treatment, payment, and health care operations. For example, we can use your Information and share it with other professionals who are treating you. We can also use and share your Information to run our business, improve your care, and contact you when necessary. In addition, Hatch can use and share your Information to bill and get payment for our services when applicable, unless you pay directly and tell us not to share your information for payment purposes.
- To provide Information provided to caregivers or relatives, with an opportunity to object to the disclosure;
- To provide limited Information provided for the purposes of research, public health, product recalls, and other safety issues;
- As permitted or required by law, including disclosures to courts and other government agencies.
Part 4: Your Rights
We will not use or share your information for any reason other than those described in this notice without a written authorization signed by you or a person legally authorized to act on your behalf (an “Authorized Agent”). An authorization is a document that you sign that directs us to use or disclose specific information for a specific purpose.
Access to your information: You may access and receive a copy of the Information that we retain on your behalf. For information that we maintain electronically, you may request a copy in a reasonable electronic format, if readily producible. Requests for access must be made in writing and signed by you or an Authorized Agent.
Amend Certain Records: You can ask us to correct or amend Information that you think is incorrect or incomplete. To make this request you should, contact us with the request as described below. We may say “no” to your request, but we will tell you why in writing within 60 days.
Requests to Limit Disclosures: You may ask us not to use or share certain Information. We are not required to agree to your request and we may say “no” if it would affect your care. If you pay for our service or product out-of-pocket in full, you can ask us not to share that information for the purpose of payment or our operations. We will say “yes” unless a law requires us to share that Information.
Receive Confidential Communications: You may ask us to contact you in a special way. You will need to ask us in writing. We will try to grant reasonable requests. Only you or an Authorized Agent may make a request related to your Information.
Part 5: Breach Notification
A breach is an impermissible use or disclosure under the Privacy Rule that compromises the security or privacy of your Information. Our Responsibilities: You have the right to be notified in the event of a breach of your Information. If the privacy or security of your Information has been compromised, we will notify you promptly with the following information:
- A brief description of what happened;
- A description of the information involved in the breach;
- Recommended steps you can take to protect yourself from harm;
- What steps were taken to investigate the breach, mitigate harm, and prevent future breaches; and
- Contact procedures so you can obtain further information.
Your Responsibilities: In the event of a breach, you are to contact the Illinois Department of Financial and Professional Responsibility at FPR@medicalcannabis@illinois.gov and the Illinois Department of Public Health at DPH.medicalcannabis@illinois.gov.
