Speak with a licensed physician about your interest in obtaining a Medical Marijuana card and becoming a Medical Cannabis Patient. If you do fit the criteria for one of the qualifying diagnoses that is listed below, your doctor will then complete their portion of your submission portal. 

STEP 2

Next, you or your provider will need to fill out the Medical Cannabis Patient Program application. This step also includes providing identification documents. You’ll need to submit a copy of your government-issued ID, two proofs of residency, and a 2×2 photo for your ID card.

STEP 3

Pay your application fee. Fees start at $25 for a one-year card for qualifying groups (see above) and $50 per year for non-qualifying groups. A two-year fee is $50 for qualifying groups or $100 for non-qualifying groups. A three-year fee is approximately $75 for qualifying groups, and $130 without. 

STEP 4

Now you’re ready to receive your card! Once you and your doctor have both filled out your respective portions of the application and paid your fee, you should be able to log into access your provisional card within 24 hours. You can begin shopping medically right away! Your approved digital card should be available within usually one week.
