If you are diagnosed with a listed qualifying condition, your doctor will then fill out a Physician Written Certification Form which you will need to submit along with your application. Please be aware the certification must show an in-person visit date within the last 90 days.

On February 19, 2019, IDPH began issuing provisional (temporary) access registration cards to online applicants. After applying, you can access & print your provisional card within 24-48 hours. It’s recommended that you call your designated dispensary to verify your card is active before driving to the physical location.
